Permanent Supportive Housing Case Manager

Position Overview: We are seeking a compassionate and dedicated Permanent Supportive Housing Case Manager to join our team. In this role, you will work directly with individuals in permanent supportive housing who have experienced chronic homelessness, with a focus on those facing complex challenges. Your mission will be to provide ongoing support to ensure residents maintain their housing and improve their quality of life.

Key Responsibilities:

Conduct thorough client assessments to identify needs and barriers.
Develop individualized case plans to address clients’ unique needs.
Provide coordinated and supportive case management services to ensure clients maintain housing and integrate with mainstream community as much as possible.
Provide transportation to off-site appointments for individual clients and for off-site group activities.
Maintain working relationship with existing agencies whose mission is serving and empowering individuals experiencing homelessness.
Plan and lead community activities for residents that increase engagement with the community and develop independent living skills.
Act as an advocate for clients to ensure their rights and needs are met.
Offer crisis intervention and immediate support as needed.
Serve as community resource relative to housing for other agencies within the Knoxville/Knox County metro area.
Utilize the HMIS (Homeless Management Information System) for recording all information relative to assigned cases.
Maintain accurate and up-to-date client records.
Educate and empower clients on tenant rights, self-advocacy, and life skills.
Willingness to be part of a rotating on-call schedule outside of regular work hours.

Qualifications:

Bachelor’s degree in social work, psychology, counseling, or related field or five years of experience working with homeless population is required.
Previous experience in case management, social services, or housing assistance is preferred.
Knowledge of local resources and social service programs.
Exceptional interpersonal and communication skills.
Cultural sensitivity and the ability to work with diverse populations.
Strong organizational and time-management skills.
Knowledge of trauma-informed care and harm reduction principles is preferred.
Compassion, empathy, and a commitment to improving the lives of vulnerable individuals and families.
Must have a valid driver’s license
